Description: ElderONE serves seniors 55 and older in the Rochester area who need nursing level or care, but can live safely at home. ElderONE provides all primary and specialty care, prescriptions, transportation to all medical appointments,homecare, therapy, and social activities at the day center or Alternate Care Sites. ElderONE is a model of choice for individuals whose focus is remaining at home and out of institutional settings. It is based on the national Program of All-Inclusive Care for the Elderly (PACE) and is one of 72 certified sites in the United States. ElderONE offers a comprehensive service package emphasizing preventative, primary, acute and long term care. ElderONE care team includes board certified primary care physicians, home care nurses and aides, physical, occupational and speech therapists, social workers, registered dieticians, RN's and LPN's caregivers and transporters. PACE is a national health care option. It is a case management model based on an inderdisciplinary team coordinating an individualized care plan for each participant. Most care is provided in a non-institutional setting; either in the day center and/or your home. The center is located on Hudson Avenue in Irondequoit. ElderONE participants attend the adult center(number of days depends on individual care plan created by family, participant and ElderONE team.) where their care team is located. Day care attendance is determined based on the individual needs of each participant.When in attendance at the day center you may see your primary care provider (PCP), physical or occupational therapists, social workers, or just come in for socialization. The activities offered in our center ranges from exercise programs, crafts, gardening and spiritual groups, to pet therapy, and music and entertainment. The ElderONE care teams coordinates transportation to all medical appointments and the center. In addition, the program provides all needed medication, medical equipment and supplies.
